Mid-Urethral Sling Procedure

A mid-urethral sling, also known as tension-free vaginal tape (TVT), is a minimally invasive procedure used to treat stress urinary incontinence. A narrow strip of synthetic mesh is placed beneath the urethra to provide support.

The procedure has been on hold in Ireland since 2018. It is a safe and effective procedure and considered the gold standard internationally in surgical treatment of stress incontinace. All cases are discussed by a Cork-based multidisciplinary team to determine eligibility, and patients are extensively counselled regarding the risks and benefits.

It is available throughout the European Union. Professor O'Reilly travells to Spain to perform the operation and provides pre operative counselling and follow up care in Ireland.

Patients considering this treatment abroad should review the information leaflet and consent documents below, and discuss the benefits, risks, alternatives, travel arrangements, recovery, and follow-up care with their clinical team before making a decision.

Treatment can be funded either privatly via healthcare abroad, or publically via the EU-Crossborder directive

Treatment Overview

When It Is Used

A TVT sling may be considered for stress urinary incontinence that causes leakage when coughing, laughing, sneezing, exercising, or carrying out other physical activities, particularly when conservative treatment has not provided adequate relief.

The Procedure

The operation is generally performed under general or spinal anaesthetic. Small incisions are made and the mesh tape is positioned beneath the mid-urethra without tension. Your surgical team will explain the exact technique planned for you.

Recovery and Aftercare

Many patients go home on the day of surgery or the following morning. Recovery varies, and your care team will provide individual guidance about wound care, activity, lifting, exercise, work, travel, warning signs, and follow-up.

Plan Before Travelling

Confirm who will provide your pre-operative assessment, surgery, discharge advice, and follow-up care. Make sure you know whom to contact if you develop difficulty urinating, fever, worsening pain, unusual discharge, bleeding, or another concern after returning home.

UGSA Surgical Treatment of SUI Pathway 2016

Select a surgical option below to explore the pathway and supporting information.

Bothersome stress urinary incontinence not responding to conservative treatment
Preferred option Possible pathway Further data required
Colposuspension
  • Superior outcomes to anterior repair, MMK and needle-suspension operations.
  • Similar outcomes to pubovaginal slings, with lower morbidity.
  • Outcomes are similar or slightly lower than synthetic mid-urethral slings, but operating time and recovery are longer.
  • A laparoscopic approach using the same technique as open surgery has similar success with less morbidity.
  • May be considered by patients wishing to avoid mesh.
Mid-urethral sling

The pathway’s recommended and most extensively researched surgical treatment, with an established efficacy and safety profile.

  • As effective as, or more effective than, colposuspension and pubovaginal sling, with less peri-operative and post-operative morbidity.
  • Includes retropubic tape, transobturator tape and mini-slings.
Retropubic tape

The preferred mid-urethral route in the pathway. In medium-term follow-up, repeat surgery for recurrent SUI is less frequent than after transobturator tape.

Transobturator tape

Short-term outcomes are similar and the operation is slightly quicker. It may be reserved for patients with a hostile abdomen or increased bleeding risk. A small proportion develop difficult-to-treat groin pain.

Mini-sling

Many products have been withdrawn and evidence for remaining devices is limited. The pathway advises use only within appropriately governed audit or research settings.

Pubovaginal sling
  • May be considered by patients wishing to avoid synthetic mesh.
  • Available comparisons suggest similar outcomes to a mid-urethral sling, but with more lower urinary-tract symptoms and repeat operations.
  • Compared with colposuspension, outcomes are similar but voiding dysfunction is more frequent.
Complicated incontinence: recurrent SUI, incontinence following radiation treatment, or cases involving a fixed urethra require individualised specialist care.
